This Privacy Notice explains how, why and where we collect your personal data.  How and where we store information, under what circumstances we would share information, how long it is stored and how you can access it.  

 

Mark Spalding, as Director of Rive Ltd (trading as MAMAHS) is the Data Controller registered with the Information Commissioners Office (ICO), the UK supervisory authority for data protection (www.ico.org.uk).  If you are not happy with any aspect of how we collect, use or store your data please contact us in the first instance, by email at elaine@mamahs.co.uk .  You will receive a response within 30 days.  If you remain unhappy about how your personal data is being processed, you can complain to the ICO.

 

Whilst we take steps to ensure your information is secure, communication by text, email, messenger, social media and our website may not be secure, so bear this in mind when using these methods.  

Collecting Information

 

During contact with you by phone, text, email, via the contact page on our website, or face to face we will collect the following information about you and your baby.  We will only collect, use and store information which is relevant and necessary for your treatment.

·      Name

·      Date of birth

·      NHS number

·      Address

·      Phone number

·      Email address

·      GP details

·      Current medication

·      Recent medical history and any other relevant history relating to you or your baby’s health.

 

If you have given us permission to take photographs of you/your baby, these will only be used in a professional/educational capacity.  They will be anonymised and may appear on our website.

 

Processing of card payments

 

We use Zettle to process card payments.  Please see Zettle Privacy Policy here https://www.zettle.com/gb/legal/privacy-policy .  Transaction data may be shared with our accountant, bank, card payment machine provider and with HMRC in certain circumstances.

How we use your personal data

 

·      To remind you of appointments and follow up

·      Audit and research

·      To contact your GP or other Health Professional (such as Health Visitor or Midwife) to inform them your baby has had a tongue tie division and any plan of care.  This is so the information can be recorded in your baby’s GP records and those relevant involved in your baby’s care are informed to ensure safe and continuing care.

 

Your information will not be shared with anyone else without your knowledge.  The exception to this is if we have any concerns about the safety of your child.  Under these circumstances we are legally and professionally obliged to share relevant information with appropriate agencies without your consent.

Storage of Data

 

We have put in place what we believe are appropriate security measures to prevent your personal data from being accidentally lost, used or accessed in an unauthorised way, altered or disclosed.

 

Any data that we keep which contains your personal/medical information and that of your baby, including summaries of our consultations and copies of emails will be stored on electronic devices which are password/passcode protected and have security software installed.  We use Cliniko, electronic practice management software to store your data (www.cliniko.com)    

Data Retention

 

We will only retain your personal data for as long as necessary to fulfil the purposes we collected it for, including for the purpose of satisfying any legal, accounting or reporting requirements.
